When “I’m Fine” Isn’t the Whole Story

During Anne Sansevero 's presentation to the New York University Langone Geriatric Palliative Care Fellow Program, she explored what hospital teams may not be able to see when planning a patient's return home.
A successful hospital-to-home transition requires us to look beyond the diagnosis and ask about function, cognition, the physical home, caregiver capacity and available community support.

For many older adults, remaining at home is one of their highest priorities. Yet the patient, family and healthcare team may have very different definitions of what "safe" means.
